MEDICINE REQUIREMENT 2018-19 For Dispensaries & Sub-Centres

Before filling Sheets, Please read the following...
1. All VASs are instructed to furnish and submit their requirement on or before 09-01-2018.
2.Kindly submit a printed copy as well.
3.VDs with Sub Centres are required to fill up the requirement for their subcentres as well, others fill the VD column only.
4.Please do not delete or try to edit any titles/ sheets. Only fill up the greencoloured cells. 5. Above the green coloured cells you will have the allottment amount which will get reduced once you start entering the requirements. The allottment should be reduced to near zero amount.

Doctors, as advised in the meeting conducted at KVK, Namakkal,on 21.12.2017 regarding Hyroponic units, persuade your beneficiary to obtain DD's as early as possible and the copy of DD along with the complete postal address of beneficiary shoud be given to Deputy Director (CBFD) through Whatsapp. ( Deputy Director - CBFD Whatsapp No. 9786377874 ).
Ensure swiftness in this regard as the beneficiary may darg on DD.
